Method:-

1. All records shall be maintained in locked file cabinets. These files will consist jointly of the health, education, social services and special needs records.

 

2. The files are available only to the:-

 

a. Head Start Director d. Education Manager

 

b. Health/Special Services Manager e. Family Workers

 

c. Family Partnerships Manager f. Auditors and Funding/Licensing Monitors

 

3. Head Start children’s files are developed to assist in planning for each enrolled family. They are the property of Passaic Family Head Start but they contain information to which parents have access. Parents may review their child’s files after making an appointment to so with the Family Partnerships Manager and may also request copies of records as needed. Whenever this occurs, the following should be observed:-

 

a) A private place to examine the records should be provided; and

 

b) An appropriate staff member (preferably a Manager) shall be present to explain and answer all questions.

 

4. No information about a Head Start’s family may be released to another agency without the prior written consent of the family concerned, with the exception of #6 below.

 

5. All Head Start staff, as well as contracted consultants, are required to treat information about Head Start families responsibly and with discretion. Staff is permitted to discuss information about families with other appropriate staff only on a “need to know” basis.

 

6. In cases of suspected child abuse or neglect, information must be released with or without prior parental permission as per the N.J. State Dodd Law.

 

7. Results:- Severe employment consequences will be enforced by the Director to any Staff members who violate the confidentiality policies of Passaic Family Head Start.
